This form should only be used by departments that do not have an OrgSync portal to publicize an event to the University community. If your organization or department has an OrgSync portal, you will create your event in the portal and request that it be posted on the Community Calendar.
This form has been created for University of Southern Indiana students and campus organizations in mind. All registered student organizations now wishing to have approval for any campus advertisement should fill out this form, and all appropriate information will be sent to the different campus departments. Please note that the filing of this form does not guarantee that advertisement will be given to the organization. Approval is still needed by the different campus department. Approval is not guaranteed.
Student groups must have confirmed their room reservation with Special Events before requesting advertisement(s).
This form should be used by off-campus advisors of USI registered student organizations to request a login to access the organization's OrgSync portal. If you are an alumni of the University of Southern Indiana, you should use your myUSI login and should not need to request a login.

Fundraising, Sales, and Solicitation are ways that organizations seek to raise funds. Whether those funds are raised to boost the personal budget or to increase funding for a philanthropy, it is important for us to keep up with the great things that your organizations are doing at USI.

All University travel by a student organization must be approved in advance of the trip and before making any financial commitments.
Prior travel approval insures the following:
the traveler's advisor and/or department approves of the activity,
funds are available for reimbursement of expenses when applicable,
travel is allowable with the funding source,
staff/faculty traveling with organization is covered under workman's compensation insurance when applicable
approved actions while on official travel are actions by a University representative; without approval the same actions could be considered as those by a private individual for which the traveler could be personally liable, and
the safety and interest of students are addressed.
This form is to be filled out every time your organization travels, including all local travel. Based upon the responses, you will be directed to any additional forms that need to be completed. You will be able to register up to four local trips that also does not include university funding.

Once you're sure that USI doesn't already have the organization you are interested in, it's time to start the process of creating a new group. Start by filling out the Intent to Organize form. This form will help you complete the first steps of starting your group.
Creating an organization from scratch is a challenging, yet fun and rewarding process. Who knows, you could create a new group that exists at USI for years to come! To start your club, complete this form and take the first step in making your mark at USI!
Congratulations! You have been nominated for a spot on the 2015 Homecoming Court.
Nominees must be current students in good standing with the university, while exemplifying leadership qualities, USI spirit, integrity, and achievement, through contributions and involvement on campus and in the community.
You now have the option of applying for court.
If you are selected for this year's court, responsibilities include a group meeting with the Senior Program Advisor in Student Development Programs on Friday, December 5, 2013, at 3:00 PM in the Student Development Programs office (UC East 1256). During this meeting, Photography Services will take group photos and individual head shots for promotional purposes. We will also discuss all the rules of being on court and how voting/selection will work.
Please note that members of the court are asked to attend homecoming events sponsored by the Homecoming Committee. The schedule of events will be given to court members at the meeting in December. Homecoming week is February 2-7, 2015. Coronation of the court will be held during half time of the Men's Basketball game on February 7, 2015.
Before submitting your online application, it is suggested that you type all answers in Word Document to prevent grammatical errors, followed by pasting your answers into the application.
The application deadline is Monday, November 10, 2014 at 5:00pm. The 2015 Homecoming Court will be announced no later than Tuesday, December 2, 2014.

Upon your request, this spring Student Development Programs will sponsor the Spring 2014 Student Involvement Fair. The involvement fair will take place Tuesday, January 20, 2015 from 10 AM - 1 PMin Carter Hall.
In celebration with USI’s 50th anniversary—we are encouraging student organizations to register for this event and showcase their organization’s history and what they are doing today. Creativity is encouraged! The organization with the best display representing the celebration of USI’s 50 years will win a grand prize. We encourage groups to creatively decorate their table, while offering games or giveaways such as food, t-shirts, cups, pens, etc.
If you are considering registering for a table, please note the following guidelines.
• Student Development will provide one 6’ x 2 ½’ table and two chairs.
• ALL tables are ASSIGNED! (The fair will have a map to assist students with table locations.)
• Open Flame, candles, water, angel hair, glitter, confetti, and straw are NOT permitted.
• NO amplified sound. Due to the location of the fair, electrical outlets will not be an option. If groups choose to have small battery operated radios, the volume MUST remain at a low level because classes will be in session.

• Selling items is NOT permitted.
Inclement Weather Policy
If there is inclement weather and the university is closed on Tuesday, January 20, 2015 the Student Organization Involvement Fair will be held on Monday, January 26, 2015 following the same schedule. Should we would also be closed on Monday, January 20, 2015, we will not hold a Spring Student Involvement Fair. You can find out if the university is closed by checking the USI homepage www.usi.edu, contact USI Emergency hotline at 812-465-1085, or listen to an Evansville media outlet.
